Population Health Managed Care Pharmacist
About the position The Population Health Managed Care Pharmacist at Ochsner Clinic Foundation is responsible for overseeing medication management for assigned patients, ensuring the appropriateness of medication therapy based on health status and institutional guidelines. This role involves educating pharmacy and medical residents, providing expert pharmacy content for various documents, and engaging in patient education and medication reconciliation. The pharmacist will also contribute to the development of best practices and guidelines for patient care, while mentoring residents and staff. Responsibilities Monitor and evaluate medication therapy for appropriateness based on patient health status and institutional guidelines. , Conduct medication reconciliation and patient education. , Provide pharmacokinetic consultation and evaluate patient response to drug therapy. , Document interventions and conduct outcome studies related to clinical activities. , Assist in developing pathways, best practices, and guidelines for assigned populations. , Educate staff, students, patients, and healthcare providers. , Mentor residents, students, and new staff to meet departmental goals. , Ensure compliance with federal, state, and local laws and accreditation standards. , Adapt communication and behavior to meet the needs of specific patient populations. , Volunteer for additional tasks during staffing shortages. Requirements Bachelor of Science in Pharmacy required. , Completion of a PGY1 Pharmacy Practice Residency or 3 years related specialty experience accepted in lieu of PGY1 Residency. , Current Pharmacy license in the state of practice required. , Effective verbal and written communication skills. , Strong leadership and teaching skills. Nice-to-haves Doctor of Pharmacy (PharmD) preferred. , Completion of a PGY2 Pharmacy Residency preferred.
