Summary ‎ We are seeking a highly skilled and empathetic Advanced Practice Registered Nurse (APRN) to join our telehealth team. This specialized role focuses on providing comprehensive memory assessment and care through a virtual platform. You will be responsible for conducting cognitive screenings managing the symptoms of patients living with dementia. A portion of this role involves managing the complex behavioral and psychological symptoms of dementia (BPSD), ensuring that both patients and their caregivers have a clear, medically sound path forward from the comfort of their homes.‎ How will you make an impact & Requirements ‎

Key Responsibilities

Virtual Assessments: Conduct comprehensive telehealth evaluations for patients with memory concerns, including detailed clinical histories and remote physical/neurological observations. Cognitive Screening: Administer and interpret validated remote assessment tools (e.g., Mini-Cog, MoCA, SLUMS) to determine the stage and type of cognitive impairment. Psychiatric Management: Diagnose and manage neuropsychiatric complications such as agitation, anxiety, depression, and sleep disturbances using both pharmacological and non-pharmacological interventions. Caregiver Support: Educate families on disease progression, safety planning (wandering, driving), and behavioral de-escalation techniques. Documentation: Maintain electronic health records (EHR) in compliance with Florida state regulations and HIPAA standards for telehealth. Qualifications & Requirements Licensure: Must hold a current, unencumbered Florida APRN license with prescriptive authority. Education: Master’s or Doctoral degree in Nursing (MSN or DNP). Clinical Experience: Minimum of 2–3 years of experience specifically in geriatrics, neurology, or a dedicated memory care setting preferred. Telehealth Proficiency: Experience conducting video-based clinical visits and comfort troubleshooting basic technology issues with elderly patients and their families. Specialized Knowledge: Working knowledge of the CMS Cognitive Assessment & Care Plan requirements and the nuances of dementia-related behavioral health. Technical Requirements Access to a private, secure, and quiet workspace. High-speed internet connection capable of supporting high-definition video conferencing. Proficiency with cloud-based EHR systems and telehealth platforms.
