Work Environment

This position is a full-time remote role based in Lakeland, Florida, allowing you to work from the comfort of your home or any location within the United States. You will spend approximately 80% of your time in the field, visiting members in their homes or in long-term care facilities, providing you with the flexibility to adapt your schedule to meet client needs. As you engage with clients directly, you will need to be patient, adaptable, and skilled in navigating various healthcare environments.

Key Responsibilities

Your day-to-day responsibilities will include:

  • Conducting thorough assessments of clients to understand their unique medical, behavioral, and socioeconomic needs.
  • Developing and implementing individualized care strategies that facilitate access to the least restrictive care options available.
  • Coordinating care plans as the single point of contact, effectively managing the continuum of care experience for clients.
  • Communicating vital health-related information among stakeholders, including family members, providers, and community resources to ensure timely and efficient care.
  • Advocating passionately for clients and their families, ensuring that their choices are fully represented within the health care team.
  • Identifying and facilitating referrals to social service programs, encompassing financial support, community services, and psychosocial support to address Social Determinants of Health (SDOH).

Patient Population / Client Focus In this role, you will specifically focus on individuals facing complex behavioral health challenges, which may include co-occurring disorders. Your work will not only address their immediate health needs but also provide support in navigating community resources such as food banks, housing assistance, and transportation options. By effectively coordinating these services, you will help empower members to achieve optimal health outcomes.

Qualifications

We require candidates who meet the following qualifications:

  • A Bachelor's Degree in a relevant field or equivalent experience.
  • A minimum of 2 years of experience in case management or social services with an undergraduate degree, or 6 years of relevant experience without a degree.
  • 1 year of proficiency with MS Office applications, including Word, Excel, and Outlook.
  • Experience in long-term care, home health, public health, or related fields is essential.
  • Ability to demonstrate experience working with local behavioral health providers and community support organizations.
  • Reliable transportation for traveling within the assigned territory to meet clients and providers.

Preferred Experience While not mandatory, the following qualifications will enhance your application:

  • Prior field-based work experience.
  • Experience in long-term care case management.
  • Familiarity with electronic health record systems and electronic charting.
  • Ability to arrange community resources effectively for clients.
  • Experience serving clients with both mental health and substance use disorders.
  • Bilingual Spanish language skills.
  • A background in managing care for populations with complex medical and behavioral needs.

Compensation and Benefits

The hourly pay for this role ranges from $23 to $41 based on your experience, skills, and local labor markets.
