Remote Care Coordinator - Behavioral Health
Our client is seeking a compassionate and dedicated Remote Care Coordinator to join their expanding Behavioral Health team. This fully remote position offers the flexibility to work from anywhere within the US, focusing on providing essential support and guidance to individuals navigating mental health challenges. The successful candidate will be instrumental in connecting clients with appropriate resources, monitoring their progress, and ensuring continuity of care through virtual means. This role is pivotal in making quality behavioral health services more accessible. Responsibilities: Conduct initial virtual assessments to understand client needs and develop personalized care plans. Coordinate and schedule virtual appointments with therapists, psychiatrists, and other healthcare providers. Serve as a primary point of contact for clients, offering ongoing support, encouragement, and resource navigation via phone, email, and secure messaging platforms. Monitor client engagement and progress, proactively addressing any barriers to care. Liaise with external agencies and community resources to facilitate referrals and support services. Maintain accurate and confidential client records in compliance with HIPAA regulations. Educate clients and their families on treatment options, available resources, and self-management strategies. Participate in regular virtual team meetings and case consultations. Advocate for clients' needs within the healthcare system. Continuously update knowledge of best practices in mental health care and remote patient support. Qualifications: Bachelor's degree in Social Work, Psychology, Counseling, or a related field. Master's degree preferred. Minimum of 2 years of experience in case management, social work, or mental health support. Experience working remotely or with virtual client services is highly desirable. Strong understanding of mental health conditions, treatment modalities, and community resources. Exceptional communication, empathy, and active listening skills. Proficiency in using electronic health record (EHR) systems and telehealth platforms. Ability to work independently, manage a caseload, and prioritize tasks effectively in a remote setting. Reliable high-speed internet connection and a dedicated, quiet workspace. Strong organizational skills and attention to detail.
