Role Description The role involves providing continuity of care by ensuring smooth transitions between care settings. Key responsibilities include:

  • Developing relationships with patients and their multidisciplinary teams to facilitate treatment and follow-up.
  • Completing nursing assessments to identify individualized patient needs and collaborating on interdisciplinary care plans.
  • Supporting patients and families through education, advocacy, and identifying barriers to care.
  • Providing expert clinical advice and designing collaborative teaching plans/programs.
  • Coordinating timely, evidence-based care across the continuum for optimal patient outcomes.
  • Evaluating and designing patient education materials and outreach programs.
  • Assisting leadership in developing program vision, goals, and objectives.
  • Demonstrating knowledge and skills necessary for age-appropriate care.

Qualifications

  • Registered Nurse license issued by the state in which the team member practices.
  • Bachelor's Degree in Nursing.
  • Typically requires 5 years of experience in nursing, including nursing assessments and care plan development.

Requirements

  • Working knowledge of quality and acute patient care needs.
  • Maintains clinical expertise to manage physical, emotional, psychosocial, and spiritual needs of patients.
  • Excellent analytical and interpersonal communication skills.
  • Demonstrated ability to work well with physicians and other professionals.
