PT / OT Home Health – Start of Care | AI-Powered OASIS (15 Minutes)
About the position Home Health Links is hiring Physical Therapists (PTs) and Occupational Therapists (OTs) for Start of Care (SOC) visits only across Los Angeles and Ventura County. We are a therapist-owned home health agency with over a decade of leadership in Southern California. We utilize AI-driven OASIS technology that significantly reduces documentation time, allowing therapists to focus more on patient care. The role offers flexible scheduling, patients located close to home, and strong clinical support. Both experienced clinicians and new graduates are welcome.
Responsibilities
- Assess the therapeutic and rehabilitative status of the patient
- Develop an appropriate treatment plan for each patient; update and modify treatment plan on an ongoing basis.
- Ensure high-quality patient care
- Use clinical reasoning in treating and planning, implementing, and monitoring patient progress
- Maintain records with accuracy and discretion
Requirements
- Complete Start of Care (SOC) assessments for new patients
- Degree from an accredited Physical or Occupational Therapy program.
- Current and unrestricted CA PT or OT license; or ability to apply
- Excellent Communication Skills
